FIELD CLOSURE PROCEDURES

This is only to be completed by a club administrative official, not a coach. USA will not consider a submission by a coach or team manager as a valid field closure.

This form is used to notify USA that a field is closed in sufficient time for USA to notify the referees and the teams. Please do not use when the game is within 3 hours or less, consider the teams travel time.

USA will use this form to notify the referee crew when possible.Clubs are responsible for also notifying their club assignor so they may notify the referee crew. USA will notify the teams, however a completed copy of this form can be sent to teams and it will be considered valid notification.
